pretty similar to the kensington expert mouse as expected, in that it is pretty good quality construction. the buttons on this one are a little weird as you're pushing in the plastic in particular spots but its not an actual separate button. also instead of a scroll wheel, you actually rotate the ball clockwise or counter-clockwise. this new scroll wheel effect takes some getting used to and tbh i preferred the old scroll wheel.
where this trackball really shines over the expert mouse though is its much lower and the angle isnt nearly as annoying, both changes are extremely helpful for carpal tunnel. you need to have your wrist unbent and straight while using the mouse in order to prevent carpal tunnel, so the weird angle of the expert mouse really sucked in that way. even with this mouse you will need to get a wrist rest that elevates your wrist if you want to use this for extended periods of time. would be nice if it came with a wrist rest which was just the correct height (1.5" high would be perfect but sadly all the wrist pads i've seen are only 1" high)

I have tried them all.
When first released, this trackball could not be programmed, & consequently was not acceptable. That has recently been corrected, & the result is an exceptionally good product. To program, you must download the free update -- very easy to do. There are more default functions than you will probably like, especially if you are a Mac user with a late keyboard. The new programmability solves all issues. Delete any other trackball drivers.
The smoothness is perfect, & 4 buttons with good tactile response + the twist-to-scroll are great. It is dust tolerant & easy to clean. Comfort is OK.
Although the older still available Expert Trackball Mouse (a misnomer) works very well, it is definitely too large & klutzy.
In my opinion, a trackball is far more efficient & user-friendly than any mouse.
The only improvement I would suggest, is to make it a little smaller -- about 85% scale.

The majority of the reviewers here love the hardware but hate the lack of software. Regarding the hardware, I will concur with most of the people who already posted reviews here: this really is a beautiful piece of hardware. It's stylish, low profile, and easy to use, although, going from an Expert Mouse, the scrolling takes a little getting used to.

Ah yes, the scrolling. To move the mouse pointer you move the ball along the X and Y axes, but to scroll your page, move it along the Z axis (that is, rotate it left or right). It works; it's low effort but doesn't feel at all cheap. The only complaint I have is that in a few years I could see the chrome wearing off the trim ring surrounding the ball (it's just plastic) from friction.

Now, about the programming: Kensington has heard your pleas, and earlier in 2010 they released a version of MouseWorks for the Slimblade here: [...]. So any of you who have given up on this trackball because of the lack of software, you may want to give it another go.

I got the Kensington Slimblade at the suggestion of an ergonomics consultant at work when I was having shoulder and wrist issues.
I'm not sure how it would stack up for video games but for office productivity I LOVE this mouse. By spinning the trackball you scroll. The buttons are easy to hit without much movement.
The only negative I had was originally you could not program the four buttons so you had to use the bottom two because the smaller top two has useless assignments. Sometimes I hit them by accident and it was annoying.

But now Kensington has available on their website a program called TrackballWorks that makes the Slimblade (and other trackballs) completely programmable! So now that I can finally program those buttons to anything I like (or make the entire left side click and entire right side right-click) it is truly awesome.

If you have larger hands and want a comfortable trackball, I recommend this one all the way.
